On line 29, the value for the givens field is "Ian P.", the suggested correction is "Ian T.". The following comment was attached: a 1st cousin once removed of mine -Name is Ian Thompson Strachan
This correction was evaluated by Mark Gallop: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
