On line 42, the value for the givens field is "Cassie", the suggested correction is "Carrie". Submitted by Margaret Cox.
This correction was evaluated by Lynda Chicorli: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct. This comment was added: I looked at the word Harrison and I think it is more so Cassie?
