On line 31, the value for the surname field is "Mash", the suggested correction is "Mashk". The following comment was attached: Possibly the "k" goes with surname instead of first name. A variation of Mask?
This correction was evaluated by Jacqui Chisamore: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
