On line 43, the value for the givens field is "Carrie", the suggested correction is "Cassie". Submitted by Shirley E. Rathbun.
This correction was evaluated by Mark Gallop: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
