On line 19, the value for the givens field is "Henesette?", the suggested correction is "Henrietta". Submitted anonymously.
This correction was evaluated by James A. Donaldson: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
