On line 42, the value for the givens field is "Henry ?", the suggested correction is "Henry Howard". Submitted anonymously.
This correction was evaluated by James Gibson: I have checked the image of the original form and decided to make a change other than what the correction suggested. This comment was added: I still find the middle name illegible, but "Howard" could be a close approximation.
