On line 1, the value for the givens field is "Mimie", the suggested correction is "Minnie". Submitted by Sandi Bellefeuille.
This correction was evaluated by Mark Gallop: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
