On line 16, the value for the givens field is "Chara(?)", the suggested correction is "Charles". The following comment was attached: Both Surname and Given name are mispelled. His name was Charles Kelman.
This correction was evaluated by Mark Gallop: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct.
