On line 10, the value for the givens field is "Samuel (?).", the suggested correction is "Samuel P.". Submitted by Ron Awde.
This correction was evaluated by Don Irwin: I have checked the image of the original form and the correct value is debatable but I think the transcribed value is more likely to be correct. This comment was added: The initial does not look like the upper case P of Prouse on line 34 or Perry on line 38 - looks more like the L on line 21 with a curve in the vertical line - but still not clear.
